Case Management Specialist – Medical Graduate

About the role

The Case Management Specialist engages patients through outbound calls using hospital data to improve retention, ensure continuity of care, and maximize patient lifetime value. The role blends patient support, medical knowledge, and a commercial mindset to encourage completion of treatment plans and use of relevant healthcare services.

Key responsibilities

• Conduct outbound calls to patients identified from the hospital database (missed follow‑ups, incomplete treatments, high‑value cases).
• Identify patient needs and recommend appropriate services, including cross‑sell or upsell aligned with medical conditions.
• Educate patients on the importance of treatment adherence and continuity of care.
• Accurately record and track all interactions in CRM systems.
• Follow up on leads and convert them into confirmed appointments or procedures.
• Maintain high levels of patient satisfaction and experience.

Required profile

• Bachelor’s degree in Medicine (mandatory); intern doctors are welcome.
• Experience in a healthcare call centre or patient coordination is a plus.
• Basic computer skills, including Excel.
