Medical Scribe – Data‑Driven Clinical Analyst for Nursing Homes

About the role

We are looking for a data‑driven medical professional to analyze EMR data and proactively identify care gaps for nursing‑home residents in the United States. The role focuses on population‑health outcomes, developing personalized care plans and supporting value‑based care initiatives rather than direct patient contact.

Key responsibilities

• Analyze large‑scale EMR and healthcare data to detect care gaps, clinical risks and intervention opportunities.
• Review resident populations to identify unmet preventive, chronic disease, medication and follow‑up needs.
• Develop evidence‑based, personalized care plans aligned with each resident’s medical history and risk profile.
• Apply clinical judgment to translate data insights into actionable recommendations for care teams.
• Stratify residents by risk and prioritize interventions for high‑risk or medically complex groups.
• Monitor population‑health metrics and evaluate the effectiveness of care plans and interventions.
• Collaborate with nurses, care coordinators, pharmacists and other professionals to optimise outcomes.
• Support quality‑improvement initiatives using clinical guidelines and best practices.
• Identify trends in hospitalisation, readmission, medication use and chronic disease management.
• Participate in designing and refining clinical algorithms, risk models and decision‑support tools.

Required profile

• Bachelor’s degree in medicine (must).
• Proven experience in a relevant full‑time medical or clinical role (must).
• Excellent verbal and written communication skills.
• English proficiency at B2 level or higher.
